P4P? Let's have C4C instead

Regarding "Insurers reach accord on P4P principles; standards still to be worked out" (Article, April 21): Everyone seems in a dither about paying doctors for their "performance" -- P4P -- recently.
We doctors can make all the recommendations and write all the prescriptions until our hands ache, but if the patient does not follow the instructions -- which most do not -- one cannot claim that the doctors are performing poorly with any accuracy. Thus, we are punished for something that is not within our control. What is new about that?
I suggest consideration for coverage for compliance -- C4C -- on the part of the patients.
