Patient-centered medical home model gains support

In April, 13 physician organizations endorsed joint principles of the patient-centered medical home at a meeting of the Patient-Centered Primary Care Collaborative, a group promoting the model's establishment.

The principles were released in 2007 by the American Academy of Family Physicians, American Academy of Pediatrics, American College of Physicians and American Osteopathic Assn.

The principles state that each patient should have an ongoing relationship with a personal physician who leads a team that takes responsibility for the patient's ongoing care.

The team coordinates care across all elements of the health care system.

The 13 organizations that endorsed the medical home principles in April: American Academy of Hospice and Palliative Medicine, American Academy of Neurology, American College of Cardiology, American College of Chest Physicians, American College of Osteopathic Family Physicians, American College of Osteopathic Internists, American Geriatrics Society, American Medical Directors Assn., American Society of Addiction Medicine, American Society of Clinical Oncology, Society for Adolescent Medicine, Society of Critical Care Medicine, and Society of General Internal Medicine.
